Video Request Forms

Provide a brief summary of the video, the purpose/objective of the video and how to video aligns with or advances the Department's mission, vision, and goals. Attach a script to this form.

List the number and type of actors, props, and other materials you may need. Ensure all potential participants have signed media release forms. If the video is an animated video, then ensure to distinguish whether you or someone else will be producing the animated content. If you'd like a third-party vendor to film and produce the video, then ensure you include a cost estimate.
